11/19/2018 2018 Combined Federal Campaign Annuitant Pledge Form


The 2018 Combined Federal Campaign (CFC) is underway. CFC has gone green and will not automatically mail CFC materials.state.gov. Instead, you may request a paper pledge form and catalog by emailing CFC@state.gov and providing your name and mailing address. A copy of the Annuitant CFC Pledge Form is also posted on this webpage.

There are three options for Annuitants sending donations this campaign year.

A. You can donate by check or money order payable to “Combined Federal Campaign” mailed and received by January 11, 2019 with total contribution outlined in section 4 of CFC Pledge Form to the following address:

B. You may also donate via monthly annuity payroll deductions for 12 months beginning February 1, 2019 through January 1, 2020. If you choose to contribute via payroll deductions please fill out and return the completed CFC Pledge Form included in the newsletter. Send the completed form by email to payhelp@state.gov or mail a copy of the CFC Pledge Form to be received by January 11, 2019 to:

Completed forms must be received for processing no later than January 11, 2019. If you have additional questions regarding CFC contributions please contact cfc@state.gov. For questions regarding payroll deductions please contact payhelp@state.gov.

C. Annuitants may also anonymously give via credit card donation via https://cfcgiving.opm.gov/welcome
